New Voluntary Health History Form – When Girl Scouts of the USA revised the registration forms, the decision was made not to include the health history information. This form is now available for girl and adult members. Release of this information is voluntary. Refer to Safety Wise for scenarios where either a health history or exam is required.
Procedures for Handling Health Information – This is a “must read” for leaders/advisors! Please be sure you fully understand the procedures. Keep a copy of this form with your other important Girl Scout documents.

Titled: "Girls Go Tech"
The Ad Council and Girl Scouts of the
USA have partnered to launch a new PSA campaign, entitled "Girls Go

mature to ensure a more diverse, dynamic, and productive workforce.
